- Transfusion orders must be documented in patient's chart.
- Consent to Transfusion Form signed by patient.
- If the patient does not have a documented blood type @ MIBH:
- A TSC (Type and Screen) must be requested by ordering in
Meditech or by using Laboratory Test Request
Form#3.
- The request for platelet products must be placed in Meditech
(PLATELET) or using Laboratory Test Request Form#3
(PLATELET). Special requirements and indications for use are
required.
- All requests must be approved by a pathologist or Hematologist
unless:
- Patient is in the OR or undergoing massive transfusion and has
a platelet count less than or equal to 100,000.
- Patient is a known Hematology/Oncology patient.
- Patient is known cardiac surgery patient.
- Product has been ordered by a designated ICU or Cardiac
physician.
