Platelet Products Requirements

Standard Requirements For Transfusion

 

Inpatient Transfusions
(Inpatient, MIB Dialysis)

MIB Clinic Outpatient Transfusions
(CTC, Prime Care, Med Clinic, ED, OPCU)

Limited Transfusion Services
(Bassett Healthcare Herkimer)

  1. Transfusion orders must be documented in patient's chart.
  2. Consent to Transfusion Form signed by patient.
  3. If the patient does not have a documented blood type @ MIBH:
    • A TSC (Type and Screen) must be requested by ordering in Meditech or by using Laboratory Test Request Form#3. 
  4. The request for platelet products must be placed in Meditech (PLATELET) or using Laboratory Test Request Form#3 (PLATELET).  Special requirements and indications for use are required.
  5. All requests must be approved by a pathologist or Hematologist unless:
    1. Patient is in the OR or undergoing massive transfusion and has a platelet count less than or equal to 100,000. 
    2. Patient is a known Hematology/Oncology patient.
    3. Patient is known cardiac surgery patient.
    4. Product has been ordered by a designated ICU or Cardiac physician.
