See Causes of Rejection for temperature requirements.

Transportation Needs

Deliver specimen to laboratory within 1 hour of collection if unspun. If not possible see "storage Requirements".

Causes for Rejection

Mislabeled or unlabeled specimen
Less than 50% draw for Vacutainer tubes
> 7 days Refrigerated
> 8 hours Room Temperature
Hemolysis > 350 mg/dL HgB
Lipemia > 900 mg/dL Trig
Bilirubin > 40 mg/dL Bili

Reference Values

Population Units Therapeutic Range Low Critical Value
Less than 1 month old mg/dL 4.2 - 9.0 < 1.0
General (> 1 month old) mg/dL 2.5 - 5.0 < 1.0

Limitations

Lipemia with triglyceride of 600 mg/dL [6.86 mmol/L] and bilirubin of 20 mg/dL [342 µmol/L] increase the PHOS result by 0.2 mg/dL [0.06 mmol/L] and 0.8 mg/dL[0.26 mmol/L] respectively, at a phosphorus concentration of 6.9 mg/dL [2.23 mmol/L].

In very rare cases gammopathy, especially monoclonal IgM (Waldeström’s macroglobulinemia), may cause unreliable results.

Eltrombopag and its metabolites may interfere with this assay causing erroneously high patient results.

Patients receiving a high-dose liposomal Amphotericin B therapy (L-AMB) may generate false low results with this reagent.

Creatine phosphate and phosphoenol pyruvate liberate phosphorus under the reaction conditions of this method and can thereby increase results.

Mannitol at concentrations of 500 mg/dL  or greater causes a decrease of 10% or more.

Additional Information

Additional Information:

Specimen tubes should be centrifuged unopened and the serum should be separated within one hour after venipuncture, as prolonged contact with red cells will cause elevated results.
