Transportation Needs

  •  Deliver specimen to laboratory within 1 hour of collection if unspun. If not possible see "storage Requirements". 
  • Aliquoted specimens must be frozen within 1 hour for transport.

Storage Requirements

  • Specimens being transported to the lab via packing list must be frozen for transport.
  • Gold top or red top tubes should be centrifuged within 2 hours of collection. Whole blood EDTA can be spun up to 24 hours.
  • Serum/Plasma samples must be aliquated and tested within 1 hour of centrifugation. If testing cannot be performed within 1 hour of centrifugation, aliquated specimen must be refrigerated until testing is performed.
  • Specimens may be refrigerated (2-8⁰C) fro up to 48 hours or frozen (-10⁰ C) for up to 3 months.
  • Specimens must be at room temperature (15-30⁰ C) when tested.

 

 

Causes for Rejection

Mislabeled or unlabeled specimen

Less than 50% draw for Vacutainer tubes

Aliquated Serum/plasma samples > 1 hour at room temperature

Aliquated Serum/plasma samples > 48 hours old, refrigerated

Aliquated Serum/Plasma > 3 month, Frozen

Clotted anticoagulated specimens

Unspun whole blood samples > 24 hours old at Room Temperature

Limitations

Interpret results along with clinical symptoms. This kit tests for IgM heterophile antibodies False positive results are known to occur due to cross-reactivity from the presence of rheumatoid factor. False negative results may occur from the timing of specimen collection relative to antibody titer peaks and troughs. A negative result should be supplemented with EBV specific serology if EBV infection is suspected.

 

As in the case of other diagnostic procedures, the results obtained by the use of this kit yield data that must be evaluated by the physician in light of other clinical information.

 

Although most patients will have a detectable heterophile level within three weeks of infection, occasionally a patient with strong clinical signs of infectious mononucleosis may take as long as three months to develop a detectable level. If further testing is desired, collect additional specimens and retest every few weeks

 

IM heterophile has been associated with disease states other than infectious mononucleosis, such as leukemia, Epstein-Barr virus, cytomeglovirus, Burkitt’s lymphoma, rheumatoid arthritis, and viral hepatitis.
