Other Requirements:
After scraping the lesion and gently applying specimen to slide, fix immediately in 95% alcohol or with spray fixative. Fix within seconds of sampling.
A completed cytology requisition is required, including pertinent patient information and site of lesion. Rush specimens require a pathologist approval.

Mislabeled or unlabeled specimen
Slide broken beyond repair.
Improper fixation or delay in fixation
Limitations
Inclusions may not be seen in up to 50% of active lesions
