Special Requirements
- Must have a completed and signed URMC requisition sent with specimens
- Print billing information to enclose.
- Place specimens, requisition, CBCA results, and billing in URMC shipping container
- Place bag in URMC pick-up area in CLP and enter information into the log sheet
- If specimens are collected at sites other than MIB campus they MUST arrive by courier by 6pm. Courier arrives for pick up at 7pm.
