Must have a completed and signed URMC requisition sent with specimens
Print billing information to enclose.
Place specimens, requisition, CBCA results, and billing in URMC shipping container
Place bag in URMC pick-up area in CLP and enter information into the log sheet
If specimens are collected at sites other than MIB campus they MUST arrive by courier by 6pm. Courier arrives for pick up at 7pm.
