Causes for Rejection

Mislabeled or unlabeled specimens
Less than 50% draw for Vacutainer tubes
Clotted specimens
Macroscopic clot detected with wooden sticks
Fibrin strands with platelet involvement on peripheral smear
Less than 250µL of sample for Microtainer tubes
Collection in 6.0 mL pink or purple top Vacutainer
Collection in any anticoagulant other that K2EDTA
> 24 hours refrigerated for CBC and/or differential
> 12 hours at room temp or > 24 hours refrigerated for reticulocytes

Additional Information:

The “DIFF” code should only be used to order a manual WBC differential without a Heme Profile. -CPT’s: 85004 and 85048 will be billed

 

If a manual differential is desired on the same specimen that has a Heme profile, “CBCM” should be hand written on the order Form. This test code is usually restricted to oncology patients and not available at all sites. -CPT: 85023 will be billed

 

If a manual differential is desired only when the specimen’s results fail the algorithm specifically designed to screen for abnormal cells, the appropriate order is for a Heme Profile w/ Automated Differential (CBCA).
