RCP's Requirements

Standard Requirements For Transfusion

Inpatient Transfusions
(Inpatient, MIB Dialysis)

MIB Clinic Outpatient Transfusions
(CTC, Prime Care, Med Clinic, ED, OPCU)

Limited Transfusion Services
(Bassett Healthcare Herkimer, OSS Dialysis)

  1. Transfusion orders must be documented in patient's chart.
  2. Consent to Transfusion Form signed by patient.
  3. Physician/Prescriber's order sheet (Red Cell Transfusion Order) in patient's chart.
  4. Current (less than 3 days old) Blood Bank specimen (red top tube).
  5. Order in Meditech or on Laboratory Test Request Form#3 requesting as RCP.  This includes a TSC and XM (Type and Crossmatch).
  6. Necessary approval by attending where noted in Indications for Usage column of the PRBC: Availability/ Comments page on this web site.
