Mayo Clinic Laboratories

Epic Test Code LAB51 Bilirubin Fractionation: Neonatal

Performing Location(s)

MIB,FOX,FTT,CRH,LFH,OCH,FXCC,HLS

Specimen Type

Serum or Plasma

Preferred Container

Gold  Microtainer

Alternate Container

Gold, Red, or Green top tube, Green Microtainer

Minumum Volume to Submit for Testing

0.5 mL Serum or Plasma

  • Multiple test can be performed from this volume. For question please contact the lab at 547-3975.

Transportation Needs

Deliver specimen to laboratory within 1 hour of collection if unspun. If not possible see "storage Requirements".

Storage Requirements

Bilirubin is extremely photosensitive. A 50% decrease in bilirubin within 1 hour has been reported for samples exposed to direct sunlight. For optimal stability, serum samples should be stored in the darkness (or wrapped in foil) at low temperatures refrigerated

Microtainer tubes should be centrifuged within 2 hours of collection. Serum from microtainer collection tubes does NOT require transfer to another tube

Sample tubes should be centrifuged within 2 hours of collection followed by transfer of the serum (red or gold top) or plasma (green top) to a labeled plastic, aliquot tube

Causes for Rejection

Mislabeled or unlabeled specimen
Less than 50% draw for Vacutainer tubes
>3 days refrigerated
 > 24 hours room temperature
> 3 montsh Frozen
Protect from light, stable 8 hours unprotected
Hemolysis > 25 mg/dL HgB
Lipemia > 500 mg/dL Trig
Hemolysis criteria does not apply of ordered as a PBIL

NOTE: The clinical laboratory no longer rejects pediatric bilirubins based on hemolysis. When detected, hemolyzed specimens with resulting hemoglobin > 100 mg/dL will be resulted and “flagged” with the comment: “Specimen hemolyzed. DBIL may be underestimated” (see Limitations of Procedure)

Limitations

Bilirubin is extremely photosensitive. A 50% decrease in bilirubin within 1 hour has been reported for samples exposed to direct sunlight

Hemolyzed specimens are generally unacceptable for direct bilirubin testing
